Make a Referral: Forms and Information
The diabetes self-management training (DSMT) and medical nutrition therapy (MNT) benefits are underutilized by Medicare beneficiaries – and anecdotal evidence suggests this is true for non-Medicare insurance providers as well. Data from the Medicare Quality Improvement Organizations (QIOs) reveals the root causes of underutilization are limited awareness and understanding of the benefits of diabetes education, and confusion about how to order the benefits for a patient.
Accordingly, AADE and the Academy for Nutrition and Dietetics have developed materials designed to provide information and assist physicians and other qualified non-physician practitioners in making referrals for diabetes education services to improve access and education to individuals with diabetes.
These materials include:
- Diabetes Services Order Form (PDF), designed to make it easy for physicians to refer for DSMT and MNT in one quick step, and
- Background information on the Diabetes Referral Form (PDF), which provides a summary of DSMT and MNT benefit requirements, as well as examples of how they can be coordinated for qualifying Medicare beneficiaries.
