Introducing the new Population Health Discussion Group

As part of the formation process for new Communities of Interest (COI), potential COIs start out as Practice Area Discussion Groups. By doing this first, AADE members have time to get the conversation and engagement of other members started, without having to focus on developing a leadership team or action plan.

We would like to introduce you to the new Population Health Discussion Group.

There is a lot of national dialogue about health systems and services incorporating a Population Health framework into the way they design and provide care for the growing number of people living with diabetes. As diabetes educators, we are intimately involved, either through our roles in delivering DSMES or perhaps for some of us, in our roles in care coordination and developing systems of care delivery for our patient population. Regardless of our role, diabetes educators are perfectly positioned to lead change and to determine the roles we play in this movement that is embracing value-based health care.
