Integration – The Path to Increasing Value and Recognition of Our Specialty

The move to value-based healthcare, where payment is made based on achievement of required clinical outcomes, raises the multidisciplinary richness of our specialty. Clinical outcomes require the application of clinical best practice combined with an informed, skilled patient in effective self-care. There is greater recognition that success lies more with what happens with the patient in between office visits rather than at the visit.​





Embracing Population Health

Population health is a model of care that effectively integrates the clinical and self-management aspects of care and where health care is headed. Population Health will be the focus of the July 2019 issue of AADE in Practice so stay on the lookout for that. There has been a lot of buzz about population health, and some of you might be wondering what it’s all about, how it impacts you as a diabetes care and education specialist, and why AADE is so interested in it. It may seem like it came out of nowhere, but the concept is not new. It is a complex, multifaceted thing with an ambitious goal of improving the health of everyone in the country where they live, work, and play. Health plans have been engaging in population health activities for decades. Now, in efforts to meet the Triple Aim of the Affordable Care Act, payers are collaborating with providers and integrating population health principles and technologies in unprecedented ways in a collaborative strategy for both groups to enhance care delivery, control the cost of care, and work together to better the health of their communities.
