DEAP Annual Status

Once programs achieve accreditation from ADCES, it is expected that the National Standards for Diabetes Self-Management Education and Support are maintained on an ongoing basis.  To ensure that programs continue to meet Quality Standards, accredited programs must submit information on an annual basis and as deemed necessary by ADCES for review.

Each accredited program is required to see at least one participant and submit outcome data annually.

FAQ

When is the Annual Status Report (ASR) Due?

The Annual Status Report is due no later than 30 days after the start date listed on the accreditation certificate. Any requests for early submission or extensions will be considered on a case-by-case basis.

Will I get a reminder?

The DEAP Coordinator will receive an automated reminder via email for the report the month prior to the program's annual start date listed on the accreditation certificate.   

Will I get confirmation that my report was received?  

Once submitted, your ASR will go under review.  The staff will contact you within 30 days if your report contains inconsistencies or there are other concerns.  Otherwise, the coordinator will be sent an approval email.

How do I report a change in coordinator? 

It is the responsibility of the accredited program to inform ADCES DEAP of any changes in DEAP Coordinator contact information.  Please submit a Change of Status request via the DEAP Dashboard to  change your DEAP Coordinator or contact information.

Will my program be chosen for a site visit at the time of my Annual Status Report?

The Annual Status Report is a form of desk-top audit, making sure you are still in compliance with the National Standards.  In addition, your program may be randomly selected for a site visit. If selected for a site visit, the DEAP Coordinator will be notified at least 10 working days prior to the audit date via e-mail.
