This presentation will describe an RDN, certified diabetes care and education specialist-led initiative to train and empower direct care RNs to provide diabetes survival skill education using mobile technology.

In this session, speakers will discuss strategies for the diabetes care and education specialist to identify disparities and therapeutic inertia around diabetes technology using the Identify, Configure, Collaborate (ICC) framework.

Advances in technology offer new device choices such as insulin delivery devices, CGM and AID systems. This presentation will provide an update of what diabetes care and education specialists need to know about these new technologies, who will benefit most and what’s in the pipeline.

Cardiometabolic Syndrome (CMS) is a recognized disease entity in 25% of adults. This session will provide an overview of CMS, the biomarkers that can predict risk and discuss types of technologies, applications and programs to mitigate risk factors.

This session will review projects funded by the Helmsley Charitable Trust involving a virtual specialty clinic and a diabetes care and education specialist supported type 1 diabetes clinic during COVID-19. Presenters will discuss mental health support and adoption of technology to support self-management.

This session will present two studies: perceptions of older adults and their care partners related to CGM data sharing through an app and whether a customized navigation system improves standard of care for pregnant women with diabetes.

This presentation will describe the ADCES technology framework Identify, Configure and Collaborate (ICC) using a program that delivers care with technology-enabled devices in high-risk communities.

This program will provide an update on currently available technology and its role in patient care as well as a look into what is expected to be FDA approved in the next 6-12 months.
