Step by Step guide to creating your application documents. You will find information about how to meet and provide documentation for each of the 6 National Standards for DSMES utilizing ADCES’s Interpretive Guidance and Additional Resources. Examples and templates can be used as a guide but you are welcome to create your own that meet the standards.

DEAP templates are available to all accredited programs and may be reproduced for general use in a clinical setting and/or education-related purposes without seeking written permission.  DEAP templates may be republished with permission from the Association of Diabetes Care & Education Specialists.
