This session will present two studies: perceptions of older adults and their care partners related to CGM data sharing through an app and whether a customized navigation system improves standard of care for pregnant women with diabetes.

This activity is designed for individual or groups of diabetes care and education specialists, including nurses, dietitians, dietetic technicians, pharmacists, pharmacy technicians, nurse practitioners, clinical nurse specialists, physicians, PAs, and other health care providers interested in staying up to date on current practices of care for people with diabetes and other related conditions.

  • Identify the barriers and facilitators to continuous glucose monitoring after using a specific app
  • Describe the SHARE intervention for older adults with type 1 diabetes and their care partners
  • Recognize a novel system for care management of women with diabetes in pregnancy
