Description

The program will overview how the MedStar Diabetes Boot Camp team diabetes care and education specialists (DCES) partnered with primary care physicians and endocrinologists to implement a comprehensive DSMES and Medication Management Program to overcome therapeutic inertia which resulted in equitable outcomes in all people with diabetes regardless of socioeconomic status or payer.

Learning Outcome

The learner will have an increased knowledge of the strategies to overcome therapeutic inertia to implement in their present diabetes care and education practice to help provide equitable outcomes in all people with diabetes.

Learning Objectives

At the end of this presentation, participants will be able to:
-- Discuss two factors that contribute to therapeutic inertia.
-- Describe the MedStar Diabetes Boot Camp, a diabetes care management program, designed to support and partner with the primary care physician and endocrinologists in diabetes with the goal of improving equitable health outcomes.
-- Describe two strategies the DCES uses to address barriers to medication taking behaviors.
-- Identify two resources available to support their efforts in the changing role of the DCES and how they can impact therapeutic inertia.
-- Describe strategies used in the MedStar Diabetes Boot Camp to break the cycle of therapeutic inertia.
-- Discuss how a DCES can implement some of the strategies discussed into their practice.

ADCES Webinars are designed for individual or groups of diabetes care and education specialists, including nurses, dietitians, dietetic technicians, pharmacists, pharmacy technicians, nurse practitioners, clinical nurse specialists, physicians, PAs, social workers, and other health care providers interested in staying up to date on current practices of care for people with diabetes and other related conditions.

Association of Social Work Boards (ASWB)

Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. The Association of Diabetes Care & Education Specialists maintains responsibility for this course. Social workers completing this course receive 1.0 continuing education credits.

Certified Diabetes Care and Education Specialists

To satisfy the requirements for renewal of certification for the Certification Board for Diabetes Care and Education (CBDCE), continuing education activities must be diabetes related and approved by a provider on the CBDCE list of Approved Providers ( www.cbdce.org). CBDCE does not approve continuing education. The Association of Diabetes Care & Education Specialists is on the CBDCE list of Approved Providers.

Board Certified Advanced Diabetes Management (BC-ADM)

ADCES is the administering body for the Advanced Diabetes Management credentials. Continuing education programs offered by ADCES can be used toward fulfilling BC-ADM Certification and recertification requirements.

Other Health Professionals

It is the responsibility of each participant to determine if the program meets the criteria for re-licensure or recertification for their discipline.
